This is a crucial opportunity to contribute to vital health support and outreach for individuals and families impacted by HIV/AIDS across Uttar Pradesh.

Your Everyday: A Role That Transforms Lives

As a Community Liaison Health Worker, you'll be key in connecting individuals with critical healthcare services. Your responsibilities will involve:

  • Ensuring Treatment Continuity: Systematically tracking cases and facilitating access to necessary medical treatment, especially for those newly diagnosed with HIV.

  • Preventing Mother-to-Child Transmission: Conducting vital follow-ups with pregnant women (PPW) to prevent HIV transmission to their babies, encouraging safe institutional deliveries and comprehensive postnatal care.

  • Promoting Child Health: Following up with infants exposed to HIV, ensuring they receive essential medication and undergo timely Early Infant Diagnosis (EID) tests for HIV-free certification.

  • Connecting to Care: Facilitating the smooth transition of newly identified reactive clients to the nearest ART Centers for immediate treatment initiation.

  • Comprehensive Support: Providing enhanced support services to PLHIV during their crucial first three months on treatment.

  • Monitoring Adherence: Regularly following up with PLHIV on various lines of ART, including those also undergoing ATT, to ensure consistent treatment adherence and effective health management. Your goal will be to help achieve over 80% treatment adherence.

  • Navigating Social Entitlements: Assisting clients with completing forms for social benefits, collecting required documents, and ensuring these are delivered to the Project Coordinator.

  • Ongoing Client Engagement: Maintaining consistent communication with stable clients to gather health updates and assess any evolving service needs.

  • Supporting Couples: Providing focused follow-up and assistance to serodiscordant couples.
