The Palliative Care Program: A Commitment to Compassionate Support
SNEHA's Palliative Care project is dedicated to providing holistic support to individuals, irrespective of their background, allowing them to live with minimal pain, dignity, and comfort until the very end. The program focuses on preventing and relieving suffering for patients with life-limiting illnesses or intense pain through medical support, counseling, and other essential services.
Key Objectives of the Palliative Care Program:
Improving Quality of Life: Enhance the well-being of registered patients through effective symptom management.
Caregiver Support: Empower primary family caregivers to confidently look after their loved ones.
Increasing Awareness: Boost knowledge and uptake of palliative care services within vulnerable communities in H West Ward.
Institutionalizing Care: Establish palliative care OPDs in four Mumbai tertiary hospitals to increase awareness, coverage, and sustainability of the intervention model.

Community Organiser Role: Your Impact in Dharavi
As a Community Organiser based at Chotta Sion Hospital, Dharavi, you will play a crucial role in the Palliative Care program's success. You will report directly to the Program Officer and contribute significantly to organizing meetings, events, training sessions, and awareness campaigns.
Key Responsibilities:
Community Outreach: Conduct regular visits to OPDs and the community to raise awareness about palliative care, identify patients, and register them for services.
Stakeholder Engagement: Collaborate with health posts, Community Health Volunteers (CHVs), ICDS staff, ASHA workers, and other SNEHA personnel to involve them in program activities.
Volunteer Coordination: Facilitate meetings with volunteers and Sanginis.
Community Mobilization: Mobilize community members for awareness meetings, events, training, and campaigns.
Reporting and Documentation: Consistently report on ongoing activities and case updates to the Program Officer. Actively participate in staff meetings, trainings, and team activities. Prepare and maintain monthly reports, training documentation, and event minutes.
Program Support: Undertake any other tasks assigned by the supervisor to support program needs.
Who We're Looking For:
Educational Requirement: Minimum SSC (Secondary School Certificate)
Experience: Minimum one year of work experience. Freshers with relevant social work experience, particularly those who have volunteered for a considerable period in the same field, are encouraged to apply.
